Hours: Primarily M-F, 8:00 am - 5:00 pm, participation in MOD rotation, some weekends for special events.Wonderful environment and strong teammate relationships! Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers Identify medically related social and emotional needs of residents, through individual and group social services programming Promote residents potential during their stay Completes psycho-social assessment, identifies social and emotional needs of residents and then develops a care plan for each resident which identifies medically related social and emotional problems,

identifies needs on discharge planning when applicable and needs for primary assigned areas Participates in interdisciplinary care plan conferences and contributes to the development of care plans and discharge planning if applicable for residents for primary assigned areas Designs and develops social service programs that meet medically related social and emotional needs of residents and complies with regulatory guidelines Assesses needs for ancillary services (dental, podiatry, optometry, psychological, etc.) and assists residents in attaining needed services for primary assigned areas Assist with interpreting and promoting Resident Rights and Resident Bill of Rights Maintain accurate and timely documentation (initial assessment, program notes, advanced directives, MDS, mini-mental State exams, depression scale, discharge documentation, etc.) for primary assigned areas Maintains Grievance Log, Satisfaction Surveys and Missing Items Log, for assigned units, and follow-up with residents for resolution, summarize monthly for quality assurance and report any concerns to Administrator of primary assigned areas Provides individualized support for residents and families in times of adjustment,

crisis or needs Orients residents and families to the facility environment and provide support in adjusting Coordinates discharge planning with teammates, residents and families for primary assigned areas Maintains professional qualifications by obtaining required CEUs annually Pursues professional development in social services disciplines through workshops, readings, and membership in appropriate organizations Provides information on hospice as appropriate and assists resident/family with initiation of services for primary assigned areas Ensure HIPAA P&Ps are being followed and report to HIPAA officer any concerns Attend all committee meetings Thoroughly investigate any complaints and concerns and report them to the Administrator Provide social service support outside primary area as needed or assigned by Administrator Other duties as assigned by Administrator

MINIMUM QUALIFICATIONS:

 Licensed by the Texas State Board of Social Worker Examiners Bachelors degree in social work (BSW), or secondary education in the social services and certification as a social work "designee may be substituted as appropriate Two years of social work supervised experience in a LTC setting preferred
LTC, Medicare, Medicaid, Discharge Planning experience required
Effective interpersonal and communications skills
Functional literacy in English

Ability to perform Essential Duties Genuine caring for and interest in the care of mature adults, people with handicapped and hospice patients
Ability to perform work tasks within the Physical Demand Requirements Ability to comply with the Resident Bill of Rights and the Employee Responsibilities
Ability to comply with the Rights of the Dying and the Employee Responsibilities
Ability to comply with organizational and departmental safety policies and procedures
