Position Summary
The Remote Medical Assistant for Chronic Care Management (CCM) will play a crucial role in our CCM program by providing essential support to patients and healthcare providers. This part-time, remote position requires a highly organized and detail-oriented individual with strong communication and problem-solving skills.
Duties and responsibilities
The Medical Assistant for CCM will work closely with the Population Health Nursing Administrator to increase access to the CCM for FHCHC Patients and to document CCM program progress in Epic. Typical duties include but are not limited to:
• Review patient medical records to identify individuals eligible for Chronic Care Management (CCM) services.
• Coordinate with FHCHC healthcare providers to ensure continuity of care and address any patient concerns or questions.
• Document all patient interactions and care plan activities in the electronic medical record.
• Maintain accurate and up-to-date patient records.
• Prepare and submit required documentation for billing and reimbursement purposes.
• Assist with data entry and reporting as needed.
Qualifications
• High School Diploma or GED and successful completion of a Medical Assistant program (diploma) from an accredited school or be a certified Medical Assistant with a minimum of one year of experience. Basic Life Support (BLS) required.
• Familiarity with electronic health records (EHRs) and Proficiency with Microsoft Office Suite.
• Strong organizational and time management skills.
• Excellent verbal and written communication skills.
• Ability to learn and adapt to new technologies and software.
